The LSA Service Delivery Model has three phases recognising that the needs and goals of Lifetime Support Scheme (LSS) participants change over time. The Acute, Transition and Living Life phases each have a dedicated team of Service Planners with a different role to play during a LSS participant’s life post-injury.

The LSA has 2 Living Life teams that currently support approximately 60% of LSS participants. As the Scheme grows, the proportion of participants supported by the Living Life teams will also increase. An updated Service Delivery Model was developed to assist with the predicted growth of the Scheme, and to ensure our service continues to remain person-centred in the long term.

One of the key innovations of the Living Life Model is the Team Response approach.

Approximately 30% of Living Life participants are engaged in activities such as work, study, travel, and other life roles. They have achieved a relatively high level of independence, have stable services, and their needs are more predictable. This group of participants are “living their lives” and only require support as needed, for example at times when their life or injury status may change.

To meet the needs of this group, a Team Response approach has been developed, which recognises the participant’s level of independence and supports their autonomy in the long term. Participants will be managed by a small team of Service Planners, rather than an individual Service Planner, who are collectively responsible for providing support when it is needed. It ensures participants have multiple Services Planners who know them and are familiar with their story. Participants are empowered to live their lives, knowing support is available when they need it.

The team will engage with these participants to review current plans or complete new My Plans, and to undertake participant check-ins and home visits. If a participant’s needs change and they require additional support, an individual Service Planner will be allocated to provide short term case management.

The development of a Team Response is a more person-centred way of working with this cohort of participants to ensure they are supported over their lifetime. We will continue to gather feedback from staff and participants on the new Living Life Model.
