Our Service Planners will involve you in decisions affecting your rehabilitation, return to work or home, goals and ongoing support needs. MyPlan gives you a way to document the progress of your main goals and the steps required to reach them. Your Service Planner will document these in your MyPlan.

What is MyPlan?

Your Service Planner will work with you to develop your MyPlan. This is a plan that is the central reference point for your approved treatment, care and support funded under the Lifetime Support Scheme.

How often will My Plan be reviewed

Your MyPlan will be reviewed with your Service Planner every 12 months. However, your MyPlan may be reviewed sooner if a significant event occurs in your life, for example:

  • return to employment
  • return to school
  • new home
  • lifetime participant accepted
  • change in health status, hospitalisation, and post-surgery.

How can I access a copy of my MyPlan

Once updated or completed a copy will either be mailed or emailed to you. You can also access an electronic copy via the LSA Participant Portal.
